WHO IS ELIGIBLE?[1]

  • Active-Duty U.S. Military Ready Reserve (Inactive National Guard, Individual Ready Reserve, Selected Reserve) and their spouses.
  • U.S. Military Veterans and their spouses within 2 years of separation from active service.
  • U.S. Military Retirees (Army, Navy, Air Force, Marines, National Guard, Coast Guard, and Active Reserve).
  • Spouses of U.S. Military Retirees (Army, Navy, Air Force, Marines, National Guard, Coast Guard, and Active Reserve).
  • Gold Star family members.

WHAT YOU NEED

  • Active-Duty: Military Leave and Earnings Statement (LES) or Defense Finance and Accounting (DFAS) Confirmation Form. Spouses must provide Military ID and a copy of their spouse’s LES or DFAS Confirmation Form.
  • Veteran: DFAS RAS Confirmation Form, Report of Separation (DD Form 214, Certificate of Release or Discharge from Active-Duty). Spouses must provide proof of relationship by an ID and a copy of their spouse’s DFAS RAS Confirmation Form, Report of Separation.
  • Retiree: DFAS Retiree Account Statement (RAS), Report of Separation (DD Form 214, Certificate of Release or Discharge from Active-Duty). Spouses must provide proof of relationship by an ID and a copy of their spouse’s DFAS RAS, Report of Separation.
  • Gold Star family members must provide one of the following: A copy of the Report of Casualty (DD Form 1300). A copy of the application for the Gold Star lapel button (DD Form 3). Or an official military service documentation supporting Gold Star status.
